Join us for STEPping Stones: Building a Bridge to Successful Transition for Individuals with Disabilities

Transition from pediatric to adult health care can be scary and confusing when you have a complex or chronic medical condition.

The STEP (Staging Transition for Every Patient) Program at Children's of Alabama and UAB is teaming with United Ability and the Alabama Department of Rehabilitation Services to answer your questions, help identify adult specialists, assist with care coordination and locate additional services in the community that fit your unique needs.

WHO SHOULD ATTEND?

• People ages 14-30 who were born with or developed a disability, and their caregivers

• Health care providers who work with people with disabilities in this age group, including OTs, PTs, COTAs, PTAs, physicians, nurses, social workers, and students in these areas
